Staff have four ways to get the COVID-19 vaccine this month.
As part of Maryland's vaccination plan, staff of Community Health Centers are in the 1A priority group and can receive free vaccinations over the next several weeks. The choice is yours and the vaccine is not mandatory, though we highly recommend it.

  1. Pre-registration is required. Sign up for a slot during these times (Note: try a different browser if the links aren't working and do not share with anyone outside of the agency): ​

  2. Bring your Health Care for the Homeless ID. Only the individual being vaccinated should report at appointment time. Any one else will be turned away.
  3. The vaccine is free. During the registration process, you will be asked to enter an insurance type, but no other info or proof is required. 
  4. Plan to stay in the waiting area for at least 15 minutes (and up to 30 if you have a history of a severe allergic reaction to anything) after getting the vaccine
  5. Sign up for your follow-up appointment (approx 28 days later). You can do this on-site. 

  6.  Consider signing up for V-Safe to monitor any side effects.

  2. Set up a MyChart account OR, if you are already a Mercy patient, log into your MyChart account here.
  3. Click “Sign Up Now” and then “Sign Up Online”.
  4. Turn your MyChart notifications ON. You will receive a notification with your appointment time once it is scheduled.
  5. Go back into MyChart to check-in and sign your consent form once you receive the notification that your appointment has been scheduled.

  1. Report to your scheduled follow-up appointment to receive your second vaccine. 
  2. Fill out the pre-vaccination form and review the Moderna Emergency Use Authorization. Bring the first two pages with you to your appointment.
  3. Plan to stay in the waiting area for at least 15 minutes (and up to 30 if you have a history of a severe allergic reaction to anything) after getting the vaccine.

Let us know about your vaccination

  • Notify your supervisor if you need to leave work to get your vaccine: As soon as you have confirmation of an appointment, cite “Sick and Safe Leave” as the reason to your supervisor. Use this leave code on your timesheet as well.
  • Notify HR: Please notify Director of Human Resources Rachelle Adams or HR Manager Michelle Hardy when you have received a vaccination. This is voluntary…and the only way for us to know what percentage of staff have been vaccinated.
