What to expect
During your first visit, we will ask you a few questions, including your name, current address/mailing address, phone number, Social Security number and insurance information if you are insured. (If you qualify for our services and have lost your identification or are not insured, we can help you get an ID and insurance.) Please have as much of this information as you can when you call to make an appointment or walk in to one of our clinics.
