As a client of Health Care for the Homeless, you can use MyHCHPortal.org—a secure online website for easy, 24-hour access to your personal health information from anywhere with an internet connection.

Set up your account
You must be a current client of Health Care for the Homeless and 13 or older to set up an account.
Four ways to register:
- Register directly through MyHCHPortal.org.
- Use the registration link that you will get by text or email two days before your appointment.
- In person: when you check-in for an appointment, ask about registering for the patient portal. Front desk staff will send you a personalized link.
- New clients can call 410-837-5533 for support.
Once on the registration page, you will need to enter identifying information and create a password:
- Name
- Date of birth
- Contact information
Please note: Each portal account must have a unique email address that is verified in our system.
We do not recommend using the mobile app for accessing the portal. It has limited functionality compared to using your browser or desktop.

MyHCHPortal.org (through our electronic health records platform, athenahealth) is a secure way to see your health information whenever you need it.

- Test results
- Medical records

Save time through Self Check-In
- Complete forms up to two days before appointments
- Confirm your demographic information
- Add preferred pharmacies
